Roots Community Health Centre ( Roots CHC) is a Surrey-based, community-governed clinic providing culturally safe, team-based primary care for immigrants, refugees, temporary foreign workers and other equity-deserving newcomers across Surrey, Delta and neighbouring municipalities.
As part of the proven Community Health Centre model, Roots CHC operates under a dual mandate providing:
1. Relationship-centred team-based primary health care that is community-governed. Interdisciplinary teams deliver seamless medical, mental-health, and wellness services.
2. Whole-life support. We assist newcomers with their social determinants of health—housing, employment, food security, safety—so families can thrive.
Our inter-disciplinary team—family physicians, a nurse practitioner, nurses, social workers, Cultural Health Brokers and an on-site pharmacist—treat the whole person, integrating medical care with language supports, settlement navigation and social-service referrals.
Help us support vulnerable newcomers as they build healthier, more stable lives in Canada.
